Arthroscopic Treatment of Acetabular Labral Tears

Abstract
Purpose: To evaluate the result of arthroscopic partial labrectomy (Group I) as compared with conservative treatment (Group II) for patients with acetabular labral tear.

Materials and Methods: Of 66 patients diagnosed as having acetabular labral tear from March 1995 to June 2001, 45 (Group I) were managed with hip arthroscopic treatment and 21 (Group II) were conservatively managed. The average follow up period was 49 (28 to 84) months. The mean age of patients was 41 (15 to 68) years. There were 29 men and 37 women. The mean duration of pain was 11 month. The most common cause of tears was degenerative, and then traumatic. We examined and compared the degree of limping, JOA pain score and individual satisfaction between the two groups.

Results: Limping score was initially 2.18 and 1.81, but dropped to 139 and 1.52 at last follow-up, in groups II, and I respectively. JOA pain scores were 0.67 (initial), 2.23 (post operation) and 2.03 (at last follow-up) in group-1, and 1.45 (initial), and 158 (at last follow-up) in group H. About individual satisfaction, group I showed 85% excellent or good, but group II showed 29% satisfied and 71% fair or poor. There were 6 cases of numbness by pudendal neuralgia after operation, but all symptoms disappeared during follow-up.

Conclusion: In group I, arthroscopic treatment produced satisfactory results on limping, pain and individual satisfaction. In group H, conservative treatment did not show effective results. Hip arthroscopic treatment was an effective method for acetabular labral tear, as evaluated through mid-term follow-up.
